Rossmore Community Rehabilitation Centre, run by City Health Care, is a purpose-built unit in Hull with 60 registered beds. It provides Discharge to Assess, a short-term stay for people who have been in hospital after an illness or injury. The aim is to help people manage everyday activities safely so they can return home, go to a previous care setting, or start a new placement within 10 days.
Assessments at the start of a stay help a therapist set realistic goals, and a shared care plan tells nurses and healthcare assistants what each person can do. Goals are reviewed regularly. A Discharge Coordinator liaises with everyone involved, and equipment, adaptations or referrals may be arranged. If more time is needed, a person may move to one of the home's intermediate care units.
